If you have trouble with the stupid survey the ask you to, just email them and tell them your, name, DOB, EDD,address,midwife and contact details, and ages of other children you have. I told them I couldnt get the survey to work and didnt have time to keep trying, they said no worries just give us all the info above and they'll get one sent :)
I had one sent when pregnant with Kayla too, very cool freebies!!!!
